 | School Nurse & Medical Info Welcome to the Ashley Hall School Nurse and Medical Information page. Fulltime Ashley Hall nurse Sallie Vee Zervos runs the infirmary and maintains student medical records. The infirmary is located in the building to the right just inside of the school's Art Center gate at the corner of Smith and Radcliffe Streets and its entrance is directly across from Burges Auditorium.
Sallie Vee also oversees faculty and staff training and certification in CPR and Automatic External Defibrillator use. All faculty and staff are certified in CPR and AED and receive scheduled refresher training.
School medical forms, required immunizations and screenings lists, and sports physical forms can be found in the downloads sections on the right side of this page. These are PDF forms and require Adobe Reader to open/print. The Ashley Hall Medical Form is a "live" form that can be filled out on-line and submitted via email.
